DISABILITY SERVICES
Due Date: June 16, 2014
Students requesting special accommodations must submit a Registration Form along with supporting documentation for approval. Accommodations cannot be made until students are officially registered with the Disability Officer (DO). Incoming students requesting special accommodations should register with the DO. TRANSCRIPTS
Due Date: July 15, 2014

Final transcripts must be sent to the Admissions Office as soon as they are available but no later than the above-noted date. The mailing address is: Admissions Office 1 Gustave L. Levy Place, Box 1002, New York, NY 10029-6574.

STUDENT HEALTH
Due Date: August 10, 2014
All incoming students and PREP scholars must complete the mandatory Student Health Requirements and return all forms to Student Health by July 1, 2014.  All forms in this Incoming Student Health Requirements packet should be submitted by email to studenthealth@mssm.edu.   The packet includes the following:

  1. Student Health Form
  2. Meningococcal Vaccine Response
  3. Consent for Provider – Patient Communication
  4. Tuberculosis, Vaccination and Titers Response Form
  5. Physical Exam Form

STUDENT INSURANCE
Due Date: August 29, 2014
The Icahn School of Medicine requires all students to have medical insurance while enrolled.  Sinai provides a medical insurance plan for all students, including optional dental and visual plans.  Students who are on the insurance plans of their spouses or parents may waive Sinai’s plan by completing the waiver form available from  the Enrollment Services Office.  More detail regarding next year’s insurance plan, along with the waiver application process will be available in the weeks to come.

Once you arrive on campus there will be a second part to the enrollment process which you’ll hear more about in the upcoming months.
Regardless of whether you are keeping your own insurance or signing up for the Mount Sinai insurance plan,  you must submit a completed Student Financial Billing Record/Waiver Form to the Office of Enrollment Services in Annenberg 12-70.

TOXICOLOGY
Due Date: August 1, 2014

Toxicology screens are mandatory before school begins. We only accept screens through Quest Diagnostics . Toxicology forms for Quest Diagnostics will be mailed to you. Once you have received your form, please search for the nearest Quest Diagnostics location. Results are automatically sent to the Admissions Office by Quest Diagnostics.

If you are an international student currently out of the country, you will complete the toxicology screening as soon as you arrive in the United States.

INTERNATIONAL STUDENTS
Due Date: August 11, 2014

The Office of International Personnel will provide the documents needed to obtain your student visa. Please fill out the F-1 student application form to begin the process to obtain your I-20. The F-1 classification is issued to students who will be pursuing a full-time or part-time study in the United States in an approved school, university or other institution. Certificate of Eligibility for F-1 student status known as the form I-20 is prepared initially by the Designated School Official (DSO – Zakariyyah Selph), from the International Office at Mount Sinai. This form reflects the program of study, dates of eligibility, financial information, etc. This is NOT the visa document. Completed applications and supporting documents may be submitted via air mail, email or fax to 212-731-7804. If a scanner is accessible a Word or PDF file may be emailed to: Zakariyyah.selph@mountsinai.org .When all documents are received, the I-20 will be provided in 1-3 weeks depending on your location. The Office of International Personnel is located on 320 E 94th Street, 5th Floor (Between 1st and 2nd avenue).

EMAIL and LOGIN
One week prior to your start date the IT Department will send you via e-mail  your unique Mount Sinai identifier and instructions on how to setup your Icahn School of Medicine at Mount Sinai e-mail account.

COMPUTER SPECIFICATIONS
Graduate Students will be required to use the SPSS software in the Biostatistics course. For graduate students (PhD and MS) in Biomedical Sciences, the Graduate School office will reimburse you for this purchase with proof of payment. Other MS/MPH students may purchase this software through their program. Students who have no laptops can use the school’s public computers for accessing resources for the laboratory component of the Biostatistics course.

Icahn School of Medicine at Mount Sinai will provide you with Symantec Endpoint software at no cost or you may use Microsoft Security Essentials. Microsoft Office Pro, Adobe Professional and other software are also available at discounted prices. The school also offers discounts when purchasing Dell or Mac laptops. Details can be found at hardware. You may also want to check the Dell website as they sometimes offer specials with greater discounts.
